| Keiichi Maebara se muda a una aldea llamada Hinamizawa y se hace amigo de cuatro chicas — Rena, Mion, Satoko y Rika. Se une a un grupo previamente formado por ellas, en donde juegan (principalmente juegos de mesa) después de la escuela. Su estadía es tranquila hasta que un hombre llamado Tomitake despierta sus sospechas sobre un asesinato en el pasado de Hinamizawa. También escucha acerca de un proyecto que pretendía construir una presa en la aldea propuesto por el gobierno cinco años atrás, el cual terminó con protestas. Al preguntarle a Mion y Rena acerca de los incidentes que acabaron con el proyecto, éstas evaden la pregunta, haciéndolo sospechar. Luego lee un artículo de un periódico antiguo, enterándose que el líder del proyecto había sido descuartizado y uno de los asesinos se mantenía prófugo. | ||||

| Durante el festival de Watanagashi, Keiichi es informado por Tomitake y una misteriosa mujer sobre la "maldición de Oyashiro". Una serie de desapariciones han ocurrido cada año durante la noche del festival y supuestamente, todas fueron causadas por la deidad de la aldea, Oyashiro. Al día siguiente, Keiichi es interrogado por un detective, Oishi, quien le informa que Tomitake se ha siucidado clavandose las uñas en su cuello, mientras que la misteriosa mujer que lo acompañaba estaba desaparecida. Luego de ésto, Keiichi se transforma en el informante de Oishi y nota cambios en las conductas de Mion y Rena, ya que siente que le ocultan cosas. Él también es acusado por parte de ellas de ocultarles algo, debido a sus reuniones secretas con Oishi. Keiichi se introduce más en la historia de la maldición y Oishi le cuenta que una de las víctimas fue Satoshi, un chico que aparentemente se había mudado de Hinamizawa justo antes de que él llegara. En ese momento, Keiichi se percata que Rena lo había estado espiando todo el tiempo. | ||||

| Asustado por la paranoia de la situación, Keiichi falta a la escuela para no encontrarse con sus amigas y profundizar aún más en los secretos de Hinamizawa. Oishi lleva a Keiichi a "Angel Mort" para almorzar y le dice que todas sus amigas están relacionadas con las víctimas de la maldición de Oyashiro. Esa noche, Mion y Rena visitan a Keiichi y le entregan una caja con ohagi (bolitas de arroz) como parte de una de las actividades del club. Una de las ohagi contenía una aguja. Keiichi está convencido de que Mion y Rena lo quieren matar. Al día siguiente, se arma con una bate de béisbol para regresar a la escuela. Rena le cuenta que Satoshi se había comportado de manera similar a él antes de desaparecer. | ||||

| Rena visita a Keiichi y se comporta de manera extraña, por lo que Keiichi la hiere dejándola fuera de su casa. Al día siguiente, Mion se disculpa con Keiichi por haberle ocultado tantos secretos en torno a Hinamizawa, pero éste la ignora fríamente. En su camino a casa, Keiichi es seguido por Rena y la confronta, diciéndole que sabe quién causó todas las desapariciones y asesinatos. Rena simplemente responde que todo es obra de Oyashiro. Keiichi escapa de Rena, sin embargo, es atrapado por un misterioso grupo de hombres y golpeado en la cabeza. Keiichi despierta en su casa junto a Mion y Rena, quienes intentaban inyectarle una sustancia con una jeringa. En defenda propia, Keiichi coge el bate de Satoshi y golpea a Mion y Rena, causándoles la muerte. Keiichi escapa de la casa hasta una cabina telefónica, en donde llama a Oishi, diciéndole que todos los asesinatos eran en realidad obra de Oyashiro, quien, según él, estaba justo detrás de él en esos momentos. | Keiichi es informado sobre la maldición de Oyashiro. Durante el ritual de Rika en la noche del festival, Shion lleva a Keiichi al Santuario de Oyashiro, donde Tomitake y su amiga, Takano Miyo, estaban esperando. Tomitake se queda vigilando el lugar mientras los tres entran a inspeccionar el lugar. En él encuentran docenas de armas de tortura y Takano explica el significado del santuario en el festival de Watanagashi. Su teoría es que los habitantes de Hinamizawa continúan realizando un milenario ritual que consiste en torturar a una persona, como sacrificio al Dios Oyashiro. Keiichi y Shion prometen no decirle a nadie que habían entrado al santuario. Keiichi se ve perturbado cuando Mion y el detective Oishi lo interrogan sobre sus actos junto a Shion la noche del festival. Shion llama por teléfono a Keiichi y le cuenta que Tomitake había clavado sus uñas en su propio cuello y Takano había sido encontrada quemada. Keiichi comienza a cuestionarse sobre su propia seguridad, así como también la de Shion. | ||||

| El alcalde de la aldea ha desaparecido. Rika muestra conocimientos sobre los actos de Keiichi la noche del festival, y en una conversación metafórica, promete ayudar al "pequeño gatito" que hizo algo malo la noche del festival y mantenerlo alejado de los "perros grandes", ya que el "pequeño gatito" lo había hecho sin malas intenciones, y alude al hecho de que Mion estaba molesta con Shion por lo que ocurrió. Shion le dice a Keiichi que le había comentado al alcalde sobre sus actos y ésta piensa que es la culpable de la desaparición del alcalde, ya que éste había prometido ayudarlos. Keiichi se preocupa por la seguridad de Rika al haberle comentado los hechos. Keiichi, Mion y Rena se dirigen a la casa de Rika y Satoko, quienes vivían juntas, pero ambas habían desaparecido. Keiichi habla con el detective Oishi y se da cuenta de que los pasos del alcalde el día del suceso no concordaban con lo que Shion le había comentado. También le informa que Shion estaba desaparecida desde la noche del festival. Esa misma noche, recibe un llamado telefónico de una chica asegurando ser Shion. Keiichi la confronta sobre su verdadera identidad. | ||||

| Keiichi y Rena sospechan que Mion está detrás de las desapariciones de Rika y Satoko, y se lo informan al detective Oishi. Luego se dirigen a la casa de Mion, enfrentándola y diciéndole que la policía no tenía suficientes pruebas para registrar la casa. Mion admite que la noche de su desaparición, Rika y Satoko habían estado con ella. Antes de entregarse a la policía, pide un momento a solas con Keiichi y Rena acepta. Mion lleva a Keiichi a una cámara de tortura. Éste ve a Shion en un calavozo justo antes de que Mion lo golpeara en la cabeza. Keiichi despierta atado a una silla, pero Mion escucha unos golpes en la puerta, suponiendo que son Oishi y Rena que veían a rescatarlo. Keiichi y Shion son salvados y Mion escapa. Keiichi es visitado por Mion, quien clava un cuchillo en su abdomen y es llevado de urgencia al hospital, donde Oishi le cuenta que Shion había caído desde su habitación, falleciendo al instante. Luego Oishi le pregunta a Keiichi si estaba completamente seguro de que Mion había clavado un cuchillo en su abdomen, a lo que responde afirmativamente. En ese momento, Oishi le revela que Mion había sido encontrada sin vida el mismo día de su rescate en el pozo de la casa. También le informa que Takano había sido quemada viva 24 horas antes de su encuentro en el santuario. Después de que Oishi abandona la habitación, Mion aparece y asesina a Keiichi. | ||||

| Keiichi's mentality is shaken when his friends tell him that he was actually at the Watanagashi festival with them, and Satoko's uncle is still abusing her, despite Keiichi being sure he is dead. He also learns that Takano went missing the night before after he saw her, and is believed to be Oyashiro's victim. Keiichi confronts Irie Kyosuke, the head doctor of the local clinic, and confesses that he killed Satoko's uncle. Upon overhearing Irie tell his assistant that Keiichi is delusional, he wishes him dead and flees the clinic. | ||||

| Keiichi goes over to the Hojo residence to ascertain Teppei's existence and finds Satoko in need of clinical help. Once he and Satoko reach the clinic however, they discover that head doctor Irie has committed suicide. Keiichi becomes increasingly paranoid as he believes that both Takano and Irie died because he wished it. At the Furude Shrine, Keiichi discovers Rika's disemboweled body, and Satoko believes that Keiichi is the culprit and flees, eventually coming to the conclusion that he is possessed by Oyashiro. After Keiichi fruitlessly tries to convince her otherwise, Satoko forces him off a bridge. Shortly afterward, disaster strikes the village of Hinamizawa, a gas leak killing thousands of residents, though Keiichi survives both his fall becomes the sole survivor of the disaster. | ||||

| Shion tricks Keiichi in a phone call and later tricks the townsfolk in order to make Keiichi a murder suspect, however she quickly realizes that Oishi suspects her instead. Afterward, she brutally murders Satoko, who she has been holding captive, only to remember to her horror that Satoshi's last request to her was to keep Satoko safe, thus driving her deeper into madness. When discovered by Rena and Keiichi, Shion lures Keiichi to the dungeon and attempts to torture him, but she hesitates and decides to let him go. After he is knocked out by using her stunner, she switches clothes with Mion and pushes her down the well, but only after Mion tells her that their grandmother may have approved of Satoshi. After being rescued along with Keiichi (because they are unaware she is the culprit), Shion starts to hallucinate and she visits Keiichi and gravely wounds him. Shion hastily returns to her apartment by scaling the side of the building, only to slip and fall to her death. This, however, makes the ending of Watanagashi-hen ambiguous, since an image of Mion appeared to Keiichi while he was in the hospital, though both Mion and Shion are dead by that point, meaning the ending is a hallucination. | ||||

She also discovers a historical case of a virus that causes whoever is infected by it to go berserk—the same disease that caused all the murders in the past. As a result, she is being hunted down by those who appear to stop at nothing to keep this conspiracy a secret. | ||||

| After learning that Mion moved the bodies from where they originally hid them, Rena's faith in her friends is shattered as she begins to go insane. Keiichi tries to dissuade Rena, but is confronted by her with his own dark secrets. After confessing his past sin to his friends, he finds that he can recall events from Onikakushi-hen, particularly his own paranoia and murder of Rena and Mion, even though they have no bearing on the current situation. This, however, strengthens his understanding of Rena's situation and his resolve to prevent such tragedies from occuring again, even going as far as apologizing to the clueless Mion for killing her. | ||||

| Her madness at its peak, Rena holds her friends and schoolmates hostage and threatens to kill them all with a homemade bomb planted on the roof of the school. Working together with the police and his friends, Keiichi manages to defuse the bomb. On the roof, Keiichi and Rena have a final confrontation, Keiichi armed with Satoshi's bat and Rena with her nata. During their duel, Rena gradually regains her sanity until she defeats Keiichi, though she is unable to bring herself to kill him. Realizing the error of her ways, Rena reconciles with her friends. Though tragedy has been averted through Rena's atonement, the constant time loops have yet to lift; however, there is now hope for the future. | ||||
